Step 1: Log In to Your Member Portal, My Health Plan 

If you have not set up your member account for My Health Plan, click the “Register” button to choose your login and password. You will need your Samaritan Health Plans’ ID card to complete registration for your member account.

Step 2: Order Your Biometric Screening Kit

From your member portal, My Health Plan:

  • Click the “Go to My Wellness Portal” link.
  • From the My Wellness site, click the “Order Biometric Test Kit” button.
  • To order your kit, click the “Home Test Kit” button.
  • Fill out the required information and select “Save & Continue."

Step 3: Complete Your Health Assessment

From your member portal, My Health Plan:

  • Click the “Go to My Wellness Portal” link.
  • From the My Wellness portal, click the “Take Health Assessment” button and follow instructions.

Step 4: Complete and Mail Your Biometric Screening Kit

When you receive your biometric test kit, you will find it contains printed step-by-step instructions as well as a supply checklist. Follow the instructions’ diagrams that show how to use the kit and mail the sample back within 24 hours.

Step 5: Receive Your Health Risk Score and Report

Approximately two to four weeks after completing your biometric screening and online wellness assessment, you will receive an email notifying you that your risk score and report are available for viewing and downloading from the My Wellness site. You will need to login to your member portal, My Health Plan, to access your confidential report and information.
