Step 1: Log In to Your Member Portal, My Health Plan 

If you have not set up your member account for My Health Plan, click the “Register” button to choose your login and password. You will need your Samaritan Health Plans’ ID card to complete registration for your member account.

Step 2: Schedule Biometric Screening

From your member portal, My Health Plan:

  • Click the “Go to My Wellness Portal” link.
  • From the My Wellness site, click the “Schedule Your Biometric Screening” button.
  • Login: rayklein
  • Password: rayklein
  • Click the “Create a New Account” button and follow instructions.
  • Note: Flu shots are offered as an option when you attend your biometric screening.

Home test kits (collect a finger stick blood sample) are available if you are unable to attend an onsite screening. Follow the instructions above to order your home test kit.

Step 3: Complete Your Health Assessment

From your member portal, My Health Plan:

  • Click the “Go to My Wellness Portal” link.
  • From the My Wellness portal, click the “Take Health Assessment” button and follow instructions.

Step 4: Complete Biometric Screening

Bring your Samaritan Health Plans’ ID card to the onsite screening. Check with your employer for screening locations and dates.

If you have chosen the Home Test Kit option, mailing instructions and materials are provided with the kit.

Step 5: Receive Your Health Risk Score and Report

Approximately two to four weeks after completing your health risk screening and online wellness assessment, you will receive an email notifying you that your risk score and report are available for viewing and downloading from the My Wellness site. You will need to login to your member portal, My Health Plan, to access your confidential report and information.
