The following navigation utilizes arrow, enter, escape, and space bar key commands. Left and right arrows move through main tier links and expand / close menus in sub tiers. Up and Down arrows will open main tier menus and toggle through sub tier links. Enter and space open menus and escape closes them as well. Tab will move on to the next part of the site rather than go through menu items.
Samaritan will be following the recent government ruling that provides members with 100% coverage for up to eight individual at-home COVID-19 tests per month. We are working with our Pharmacy team to ensure these tests will process at no cost to you. We hope this is ready by Jan. 15. In the event it is not, you may submit any tests purchased on or after Jan 15, for reimbursement to the plan. Please keep an eye on our website as that will have the most up-to-date information.
If you purchased an over-the-counter item before you received your benefits card, your card did not work, or an item was not covered when you think it should have been, you can request reimbursement by completing this form.
Do you need to replace a benefit card that was lost, stolen or not received?
Go to MyHealthPlan.samhealth.org, log in and then click on the button that says, “Benefit Card Login”. Once in the portal, you can report the first card sent as lost or stolen and a new card will be mailed to you. Another option is to contact Customer Service and request a new card be mailed.

By completing a Health Risk Assessment (HRA), you will get a customized care plan that can give you extra help in areas where you need it most.

Getting answers to questions you have about your health or treatment.
Finding out what you can do to feel better.
Identifying areas where additional support can be offered by completing the annual Health Risk Assessment.

Coordinating follow-up with specialists or other services.
Understanding and following instructions from your doctors and other health experts.
Access to a 24/7 nurse advice line if you are sick, hurt or have an urgent health question.
