Preventive Care Can Be Rewarding!
Getting the care you need can help you stay healthy. Samaritan Advantage Health Plans members can now earn rewards just for completing preventive health services.
Earning rewards is easy! Visit your provider, receive one of the eligible preventive services and once your provider submits a claim to us, we will reward you.
You will receive your Rewards & Incentives MasterCard after your first service which earns a reward. Each time you earn new rewards, your card is automatically loaded with the earned amount.
Which Preventive Services Earn Rewards
The Following Preventive Services Are Eligible for Rewards*:
- Health risk assessment completion (Samaritan will call you once a year to take the assessment) — $25.
- Annual Wellness Visit, annual physical exam or Welcome to Medicare Visit (one per calendar year) — $25.
- Flu vaccination (one per calendar year) — $25.
- Cancer screenings: breast, cervical, colorectal, prostate (one screening per calendar year) — $20 per screening.
- Bone density measurement screening (one per calendar year) — $20.
- Glaucoma screening (one per calendar year) — $10.
- Diabetes screening (two per calendar year) — $10 per screening.
*Members may not be eligible to earn every reward. The frequency of needed/recommended preventive services is based on medical guidelines. Not all preventive services are medically appropriate each year. Samaritan Advantage Health Plans uses frequency guidelines adopted by the Centers for Medicare and Medicaid Services and the U.S. Preventive Services Task Force. It’s Time to Spend Your Rewards!
Go shopping! Use your card to redeem rewards at grocery stores, pharmacies and home improvement stores.** Some examples of retailers in your area are Safeway, Fred Meyer, Walmart, Samaritan pharmacy locations, Lowes, Home Depot, Ace Hardware and many other retailers. Use it to also pay education and health care expenses.
Use your rewards before they expire. Rewards earned within a calendar year will expire on June 30 of the next year (or 90 days after your coverage with Samaritan Advantage Health Plans ends).
Use your card to redeem rewards and make purchases. You cannot submit to be reimbursed under this program.
Your card can be used at many retailers. However, it cannot be used to buy alcohol, tobacco, firearms or at/for:
- Restaurants and bars.
- Liquor, cigar, automotive part and service, beauty supply or jewelry stores.
- Movie theaters.
- Gas pumps.
- Insurance payments.
- Transportation.
- Golf courses, bowling alleys and other recreational activities.
**Some exclusions apply. Please see your card upon receipt for specific exclusions.
Frequently Asked Questions
Rewards can be earned for visits or screenings on or after Jan. 1, 2023.
You will receive your rewards and incentives card after we get a claim for the first service that is eligible to receive a reward. It may take 30 days from the date of your service before you receive your card. Samaritan must wait for your provider to bill us. Your card will be mailed in a plain white envelope, so please watch your mail so it is not thrown away by mistake.
You must use your rewards and incentives card to pay for a purchase at a covered retailer. There is no option to submit for reimbursement on this program.
Samaritan has to wait for your provider to bill us. Please allow at least 30 days for a reward to be added to your card.
